Please fill out this form as completely and accurately as possible so we can get to know you and your pet(s) before your visit.

-Please note: you will be charged an additional day of boarding if your loved one is not picked up before noon.

It is the policy of Animal Medical Center of Redlands to provide conscientious, affectionate, individual care for each animal left in our trust. We will provide clean, sanitary, and safe quarters. Animals will be fed regularly according to the owner’s instructions. Any special medications will be administered as directed. Animal Medical Center of Redlands, its agents, and employees will exercise reasonable care to prevent loss, injury, illness, and death of animals in their care. All pets are handled humanely and in accordance with Sec. 1834.5 of the California Civil Code.

Vaccines
To protect your pet and our other guests, his/her rabies vaccine must have been given in the last 12 months if he/she is under the age of two or the last 36 months if the pet is over the age of two. All other vaccines must have been administered by a veterinarian within the last 12 months. If a pet does not receive his/her vaccines at this facility, you must show documentation from a veterinarian who verifies current vaccinations. If any vaccines are past due, your pet must be vaccinated before boarding for his/her protection. Vaccines administered at this facility will be added to your bill.

Canine

  • DAP+4L
  • Bordetella
  • Corona Virus
  • H3N2
  • Rabies

Feline

  • FVRCP
  • Feline Leukemia
  • Rabies

Recommended, but not required for boarding:

  • Lymes
  • FIP

Diet:
We feed our canine guests Science Diet Sensitive Stomach Food. Our feline guests enjoy Science Diet Feline Maintenance food. If you prefer, we will be pleased to feed a prescription diet or another commercial diet of your choice. You may purchase this diet from our hospital (Hills & Royal Canin) or supply it yourself.

Medication
If your pet is on medication(s), we will be happy to have trained medical personnel administer it as per your instructions. There is a small treatment fee for this service. Please bring appropriate medications and provide our team with detailed instructions.

Check-Out Time:
Check-out time is 12:00 noon. Pets admitted will be charged for the day of check-in. Pets discharged after check-out time will be charged for the day of checkout. A minimum of one day will be charged. Pets will be admitted and released only during normal business hours. No exceptions.

Emergency Care:
It is the policy of the Animal Medical Center of Redlands to treat any medical problems that may cause unnecessary discomfort or become a threat to the life or health of a pet. In such an event, a reasonable attempt will be made to contact you. However, if we are unable to contact you, we will initiate such treatment as deemed necessary by the staff veterinarian without your prior approval. Medical expenses incurred shall be the responsibility of the owner for any illness or injury beyond the reasonable control of the Animal Medical Center of Redlands.

WE PRIDE OURSELVES ON HAVING A FLEA-FREE FACILITY. IN ORDER TO MAINTAIN THIS STATUS, WE WILL ADMINISTER AN ORAL FLEA TABLET CALLED CAPSTAR TO EACH PET WHO ENTERS. THIS TREATMENT WILL BE GIVEN AT THE OWNERS EXPENSE.

Holiday Surcharges:
Throughout the holiday season, it is necessary for the Animal Medical Center of Redlands to attach a holiday surcharge, due to the large volume of boarding pets we must increase our staff at holiday rates in addition to overtime. There will be an additional fee of $5.00 per pet per day attached to the following dates: July 3rd-7th, November 20th -27th & December 20th -January 5th.

Special Needs Pets:

  • Fractious pets will be charged an additional fee of $38.67 per day.
  • Senior pets requiring special attention will be charged an additional $15.50 per day.
  • Diabetic pets will be charged an additional $7.43 per day.

Puppies/Kittens are allowed to board after having received the first two sets of vaccines.

I agree to pay the invoice in full to the Animal Medical Center of Redlands at the time of discharge. I certify that my pet appears to be free of contagious disease and has not bitten anyone within the past ten days. I understand that if I fail to pick up my pet within ten days of notification to the above address, my pet will be considered abandoned and will be handled in accordance with state law and that doing so does not relieve me of my financial obligations.

The owner agrees that by executing this agreement, the provisions hereof constitute a continuing agreement between the Animal Medical Center of Redlands and the pet owner for any subsequent boarding.
